Despite frequently being compared online, CoolSculpting and AirSculpt aren’t two versions of the same treatment.

CoolSculpting is a noninvasive fat-reduction procedure that uses controlled cooling.

AirSculpt is a minimally invasive liposuction procedure in which a cannula is inserted beneath the skin and fat is physically removed.

What Is CoolSculpting?

CoolSculpting uses a technology called cryolipolysis, or controlled cooling, to target subcutaneous fat cells without surgery.

An applicator is placed on the outside of your skin and precisely controlled cooling targets the underlying fat.

Over the months following treatment, your body gradually clears the affected fat cells.

There’s:

  • No incision.
  • No cannula.
  • No anesthesia.
  • No fat physically suctioned from your body.

CoolSculpting and CoolSculpting Elite are FDA-cleared for visible fat bulges in multiple areas including the abdomen, flanks, thighs, upper arms, bra fat, back fat, banana roll and beneath the chin and jawline.

What Is AirSculpt?

This is where some of the confusion begins.

AirSculpt is sometimes described as an alternative to liposuction, but technically it is a proprietary method of liposuction.

AirSculpt uses small access points through which a cannula is inserted beneath the skin. The cannula mechanically separates fat while suction physically removes it from the body.

Patients generally remain awake and receive local anesthesia rather than general anesthesia.

So AirSculpt may be less invasive than some traditional surgical liposuction approaches, but it is still an invasive fat-removal procedure.

That’s an important distinction if avoiding an invasive procedure is one of your priorities.

Which Removes More Fat: CoolSculpting or AirSculpt?

Here’s where we believe honesty matters.

AirSculpt can potentially remove substantially more fat during a single procedure.

That’s because the surgeon is physically suctioning fat from the body.

CoolSculpting works differently.

The goal is controlled, progressive contouring of targeted fat deposits without entering the body surgically.

CoolSculpting offers measurable fat reduction without an invasive procedure.

And that’s the tradeoff patients should actually be considering.

But Doesn’t AirSculpt Have FDA Clearance Too?

Yes and this deserves some nuance.

The AirSculpt Body Contouring System received FDA 510(k) clearance as a suction lipoplasty system for removal of tissue or fluid during general liposuction procedures for aesthetic body contouring.

Interestingly, according to the FDA’s clearance summary, clinical data were not required to establish substantial equivalence for that clearance.

That doesn’t mean AirSculpt doesn’t work.

Liposuction itself has decades of clinical experience behind it.

It simply means patients shouldn’t interpret “FDA-cleared” as evidence that the FDA conducted a clinical trial proving AirSculpt produces better results than CoolSculpting.

No high-quality head-to-head clinical trial currently shows that AirSculpt is superior to CoolSculpting, or vice versa.

They are fundamentally different procedures.

What About the Research Behind CoolSculpting?

This is one area where CoolSculpting stands out.

Researchers have studied cryolipolysis for well over a decade.

A systematic review of the early literature found an average 19.55% reduction in subcutaneous tissue among patients with objective measurements.

More recently, a 2025 systematic review and meta-analysis evaluated 30 studies involving 3,158 participants and found statistically significant improvements in several measures of adiposity following cryolipolysis. Overall patient satisfaction was approximately 80%.

For us, that matters.

We aren’t interested in simply adopting whatever body-contouring technology happens to have the best advertising campaign this year.

Is CoolSculpting Risk-Free?

No.

And any provider telling you otherwise isn’t giving you the full story.

Temporary effects can include redness, swelling, tenderness, tingling, numbness and discomfort.

CoolSculpting also carries a rare but important risk called paradoxical adipose hyperplasia (PAH), where the treated fat enlarges instead of shrinking and may require surgical correction.

Fortunately Allergan, the company who owns CoolSculpting, has an assistance program and offers up to $7500 in financial compensation if you devlope PAH and are diagnosed by a surgeron.

We believe patients deserve to know that before choosing treatment.

Is AirSculpt Risk-Free?

No.

AirSculpt’s smaller entry points and avoidance of general anesthesia in typical cases don’t change the fact that a cannula is entering the body and fat is being physically removed.

As with liposuction procedures generally, patients need to discuss potential complications, recovery, candidacy and their individual health risks with the surgeon performing the procedure.

That’s another fundamental difference in this comparison.

CoolSculpting is noninvasive. AirSculpt is minimally invasive.

Neither description is inherently “better.”

It depends on what you’re comfortable with.

Who Might Prefer AirSculpt?

We don’t think everyone should choose CoolSculpting.

AirSculpt may make more sense if:

  • You want a single session
  • You’re comfortable undergoing an invasive procedure.
  • You’re comfortable with local anesthesia and potentially sedation.
  • You’re willing to accept recovery and post-procedure care.
  • You want a more dramatic one-session change.
